Mammogram Reminder

An annual mammogram is an important part of a woman's breast health. Together with monthly breast self-exams and annual clinical breast exams performed by a healthcare professional, annual mammograms beginning at age 40 can increase the chance that breast changes will be detected early.

Mount Carmel can help you remember to schedule your annual mammogram. We'll send you an e-mail message on the first of the month you request as a reminder to schedule your mammogram. If you've had a mammogram in the past year, you may want to be reminded to make your next appointment for the same date. Some women prefer to use their birthday as a yearly reminder to schedule their mammogram. Either way, if you'll fill out the form below we'll sign you up, send your reminder and help ensure your future breast health.
