- Mount Carmel New Albany
- $500 (1 medical staff)
- $800 (2 medical staffs)
- $1000 (3 medical staffs)
No. Once the verification process begins on your application, the fee cannot be refunded.
The CAQH is the “Council for Affordable Health Care” application. We utilize the CAQH application as part of the credentialing process, which, according to H.B. 125, The Healthcare Simplification Act, is the only acceptable application for physician credentialing and reappointment in Ohio. No. It is not the policy of the CVO to expedite one application over another. Applications are processed in the order they’re received.
Yes. Providing you have interviewed with your Department Chair, your application is approved by the Credentials Committee and there are no concerns with your application, you will be sent a letter confirming you have been granted “temporary” privileges immediately following the Credentials Committee meeting. Permanent privileges are granted once your application has been presented to the quarterly Credentialing Board of Trustees for approval.
Once you receive your “temporary” privileges letter OR once you receive your final approval letter from the Board of Trustees.
