The physician planner must complete this form by describing the details of the proposed CME activity. Requests are due at least 60 days prior to a one-hour activity and 4-6 months prior to major symposia.
When commercial support is received by Mount Carmel for a CME activity, a Letter of Agreement must be signed by both commercial supporter and Mount Carmel's designee stating the terms, conditions, and purposes of the commercial support.
All planners, teachers, and authors involved in the development of CME content must disclose to Mount Carmel's designee their relevant financial relationships (any amount occurring within the past 12 months that creates a conflict of interest) with any commercial interest. This form must be completed, signed, and reviewed for potential conflicts of interest prior to the CME activity.
