Forms

Your role in a CME-accredited activity is vital to the success of the activity and serves to facilitate compliance with the Ohio State Medical Association (OSMA) regulations. To assist you in the planning and implementation of successful CME activities, the following forms may be accessed:

Category 1 CME Credit Application

The physician planner must complete this form by describing the details of the proposed CME activity. Requests are due at least 60 days prior to a one-hour activity and 4-6 months prior to major symposia.

Letter of Agreement

When commercial support is received by Mount Carmel for a CME activity, a Letter of Agreement must be signed by both commercial supporter and Mount Carmel's designee stating the terms, conditions, and purposes of the commercial support.

CME Disclosure Forms

All planners, teachers, and authors involved in the development of CME content must disclose to Mount Carmel's designee their relevant financial relationships (any amount occurring within the past 12 months that creates a conflict of interest) with any commercial interest. This form must be completed, signed, and reviewed for potential conflicts of interest prior to the CME activity.

How to Write CME Objectives

Writing objectives for CME activities can be challenging. The below form is intended to assist CME planners and speakers in writing objectives which meet CME requirements.
