| Your role in a CME-accredited activity is vital to the success of the activity and serves to facilitate compliance with the Ohio State Medical Association (OSMA) regulations. To assist you in the planning and implementation of successful CME activities, the following forms may be accessed:
CME Activity Planning Form
The physician planner must complete this form by describing the details of the proposed CME activity. The form must be submitted to the CME Committee for review and approval at least 60 days prior to the activity.
Letter of Agreement
When commercial support is received by Mount Carmel for a CME activity, a Letter of Agreement must be signed by both commercial supporter and Mount Carmel's designee stating the terms, conditions, and purposes of the commercial support.
Faculty and Planning Committee Member Full Disclosure Form
All planners, teachers, and authors involved in the development of CME content must disclose to Mount Carmel's designee their relevant financial relationships (any amount occurring within the past 12 months that creates a conflict of interest) with any commercial interest. This form must be completed, signed, and reviewed for potential conflicts of interest.
Planning Committee Disclosure Form
All decisions in the planning and implementation of a CME activity must be made by Mount Carmel designees and without the control of commercial interests. This form must be signed by all planning committee members verifying independence, balance, and objectivity.
