Health Partners

Mount Carmel Health Partners is a physician-hospital organization (PHO) dedicated to transforming the delivery of health care to meet the changing needs of our patients and the communities we serve.  Health Partners is a jointly-owned venture between Mount Carmel Health System and over 1600 primary care and specialty physicians. It is our mission to venture collaboratively to provide high quality, cost effective, coordinated, and innovative patient care in all health care settings and to be the premier health care provider network in central Ohio.

Clinical Integration

The Health Partners Clinical Integration program enables primary care physicians and specialists to work more closely with one another in a coordinated manner to provide a higher standard of care that is effective, efficient, and most importantly, patient-focused.

 Clinical Integration is foundational because it:

  • Demonstrates improved clinical outcomes and evidence-based care for patients.
  • Reduces health care costs for patients, employers, and health plans.
  • Enhances the coordination of care between physicians, hospitals, and other health care providers.
  • Facilitates the delivery of the right care at the right time in the right setting.
  • Improves reimbursement through demonstration of quality and cost improvement.

A critical element of the Clinical Integration program is data reporting.  To meet this need, Health Partners offers the McKesson Population Manager, a patient registry system, to support physicians in tracking quality measures to reduce gaps in care. The reporting of this data allows physicians to capitalize on financial incentives for their quality performance.  Health Partners supports the implementation and ongoing maintenance for this system.

Quality Initiatives

Quality is the foundation upon which Health Partners is built and is the cornerstone of our success.  Health Partners actively engages member physicians in our efforts to demonstrate the quality of care we provide while demonstrating value to patients, payors, and employers in our community.  Active involvement with quality programs involving multiple payors has resulted in financial rewards for our physicians who participate in the Clinical Integration program. Our strength in the future will be based on the ability of our network of hospitals and physicians to continue to build on the successes of the past and collaborate in new and innovative ways.

Access to Managed Care Contracts

As a member of Health Partners, physicians have access to numerous health plans, including our exclusive contracts with MediGold and the Trinity Health Plan. The Health Partners Provider Relations team helps bridge the gap between member physicians and third-party payors by serving as a liaison and assists members with questions regarding our managed care contracts.

Credentialing and Network Development

The Health Partners Network Development team is the point of contact for all eligibility, membership, credentialing, and recredentialing questions. A distinct benefit of membership is that Network Development offers members a comprehensive credentialing process. Third party payors have delegated the credentialing function to Health Partners, creating ease of participation for physicians by not needing to complete paperwork for each payor.  Network Development has also streamlined the recredentialing process which takes place every two years after initial credentialing by working with the MCHS Credentials Verification Office (CVO), which saves our members a great deal of time and effort.

Education and Communications

Health Partners offers its members ongoing CME opportunities, office staff training seminars, and educational webinars throughout the year. Since communication is a key to success, Health Partners publishes two bimonthly member newsletters and an annual report, Commitment to Quality.  Additional value-added services include our Discounted Professional Services Program, which offers our members discounts and services that benefit their practices, and the Semi-Annual Vendor and Payor Fair, which provides the opportunity for physicians, practice managers, and office staff to visit with our Discounted Professional Service vendors as well as representatives from managed care health plans.
