With the help of the professionals at the Mount Carmel Heart Failure Centers, it can mean learning to live with your condition without letting it put unnecessary limits on your life.
Under the direction of a cardiologist, the experienced advanced practice nurses at our Heart Failure Centers can provide you with education, prescribe and monitor your medications, and offer regular follow-ups. And the process works. Recent outcome data show readmission rates for active Mount Carmel Heart Failure Center patients are just 6% — well below the national rate of 24.7%.
Mount Carmel has three Columbus-area Heart Failure Centers to choose from, and appointments and a physician referral are required.
