Our heart failure coordinators are experienced cardiac nurses who work with you, your family or caregivers, and your physicians to facilitate treatment based on your needs. In addition to reviewing your medical history and identifying changes in your condition and treatment, they also make sure you have access to the education and resources you need.
By grouping our heart failure specialists and technologies together into Heart Failure Units at Mount Carmel East, West and St. Ann’s, we give you the most direct care possible and surround you with expert professionals who understand heart conditions and symptom management.
Mount Carmel’s three Heart Failure Centers are dedicated to helping you learn to live with heart failure. Under the direction of a cardiologist, the experienced nurse practitioners at our centers can provide you with education, prescribe and monitor your medications and offer you regular follow-up. The centers are located in Westerville, downtown and on the east side of Columbus. Hours vary by location, and appointments are required. For more information about our centers, click here.
Exercise is important to your recovery, and supervised exercise is a great choice for heart failure patients. Mount Carmel’s Healthy Exercise & Lifestyle Program (HELP) not only gives you access to our multidisciplinary staff while you work out, but also gives you help in identifying risk factors and making lifestyle changes.
Keeping your symptoms under control is easy with Mount Carmel’s Heart Failure Tel-Assurance® program. It allows you to make a daily phone call to our automated service, share key information with our Heart Failure Services team, and receive a call if they suspect any early signs of heart failure. It’s a quick and easy way to make sure you get the care you need as soon as you need it.
In addition to all these valuable programs and resources, Heart Failure Services can also provide you and your family with assistance when it comes to home care, transition to extended care facilities, and palliative care should you need it.
