Anticoagulation Center
The Mount Carmel Anticoagulation Center is a service established to carefully monitor and manage the medication that patients take to prevent blood clots. Monthly, patients receive a blood test to ensure stable and effective levels of Coumadin®(warfarin) as well as a review of all other medications that could potentially interact with their Coumadin®(warfarin) therapy. The center is staffed by registered pharmacists with specialized training in anticoagulation and also offers patients blood pressure checks and weight analysis.

A physician referral is required, and patients must meet with the clinical pharmacist at least once a month (more often when first starting Coumadin® (warfarin) to ensure the correct dosage).
