To be
eligible for application for resident position, the applicant must, by the
intended date of admission:
For
application to PGY-1 Pharmacy Practice Residency interested candidates should
submit the following requirements by January 1st to eligible for
consideration.
- Letter of Intent – one page
letter explaining why the candidate is interested in the position and what
qualifications he/she has that are not fully captured within the remaining
application packet
- Curriculum vitae
- College transcript(s)
(optional)
- Three letters of
recommendation
An on-site interview is required but not guaranteed to
all applicants and will be arranged after application complete.

Ohio State University, and for the residency at any or all of the five

Application, they give you the option to check each hospital with which you
