New Patients

So much of what makes visiting your family care physician a pleasant experience is convenience. Struggling to get an appointment and waiting to see your physician are two challenges you might have faced in the past. At HealthProviders, we try to keep things convenient and simple. That's why we offer new patients like you the opportunity to save yourself some time before your first visit. Below you'll find a few forms that you can download, fill out and present when you arrive at the physician office. They'll make your visit a faster, more efficient and pleasing experience for you and your family.

Scheduling Appointments

Always call to schedule an appointment. Making office visits as convenient as possible for all our patients means we can't typically accommodate walk-ins. If you're sick and need to schedule an appointment immediately, call our office first thing in the morning. We'll make certain to address your concerns. And be sure to tell the scheduler why you're coming in so your physician can set aside the appropriate amount of time to see you.

After Hours

Should you need to contact our office during the evening or on weekends, please call the daytime phone number. While we encourage you to call our office during regular business hours, we understand that health emergencies can happen at any time. We always will have physicians on call to help you in the event of an emergency.

Prescription Refills

To ensure you can get your prescriptions when you need them, we suggest making renewal requests during your regular doctor visits. If you don't have enough medication to get through to your next appointment, you can request refills by calling your physician's office between 10:00 a.m. and 11:30 a.m. and between 2:00 p.m. and 4:00 p.m. weekdays. When you call, please provide your pharmacy's phone number. We'll call the pharmacy with your prescription in 24-48 hours. Don't plan to make refill requests during evenings, weekends and holidays when our office is closed.
Please bring in all of your current medications, including over-the-counter and herbal medications, with you to your regular doctor visit.

No Show Policy

For the convenience of all of our patients and staff, we require 24 hours' notice whenever you need to cancel an appointment. Getting cancellation information in advance allows us to schedule and serve other patients — in some cases that may be you or a member of your family.

We recognize that unforeseen circumstances can arise and appointments cannot be cancelled in advance. With that in mind, we'll forgive a single missed appointment. However, after one miss or late cancellation you'll be charged $25.00 for each instance. We reserve the right to dismiss patients from our practice after three missed appointments in a twelve-month period.

Release of Medical Records

All requests for release of medical records must be in writing and will be processed within 30 business days. The medical release form is available in our physician offices or can be downloaded here.

Disability Forms & Worker's Compensation

Disability forms and worker's compensation paperwork will take a minimum of 3-5 business days to complete. We reserve the right to charge for completion of these forms and may require a physician visit before completion.

Payments

In an effort to keep fees down, payment is expected at the time of service. Co-payments are collected upon arrival, prior to your visit with the physician. We accept cash, credit or debit cards, and checks. If you have a balance, payment is required within 30 days unless arrangements have been made with the Office Manager in advance. Insurance is not a substitute for payment. Insurance companies are billed directly. However, you are responsible for co-payments, deductibles and non-covered services at the time of service.
