Offering inpatient and
outpatient services, the Mount Carmel Heart Failure Program provides a
comprehensive approach to heart failure disease management. The program
provides education, personal care and access to a host of helpful resources.
Heart Failure Coordinators
Heart Failure Coordinators put patients in touch with information and resources
to facilitate their treatment while in the hospital.
Heart Failure Units
By grouping heart failure specialists and technologies together into Heart
Failure Units at Mount Carmel East and West, we provide patients with the most
direct care possible and surround them with expert professionals who understand
heart conditions and symptom management.
Heart Failure Center – Now Open!
The Mount Carmel
Heart Failure Center is dedicated to helping patients learn to live with
heart failure. Under the direction of a cardiologist, our experienced nurse
practitioner provides education, prescribes and monitors medications and offers
regular follow-up. The clinic is located within National Road Family Health and
is available Tuesday through Thursday from 8:30 a.m. – 3:30 p.m. by
appointment.
