The Mount Carmel Pharmacy Outpatient Care Center (POCC) is the place to get the help you need from a Pharmacist to carefully monitor and manage your health. Staffed by a team of clinical pharmacists with specialized training in multiple disease states. Our centers can provide you with:

Anticoagulation management

Are you on Warfarin? Tired of the venous draws to manage your labs?

Are you on an anticoagulant other than warfarin? Such as Apixaban (Eliquis), Dabigatran Etexilate (Pradaxa), Edoxaban (Savaysa), Rivaroxaban (Xarelto)?

For Warfarin patients, we offer point-of-care testing utilizing only a fingerstick and a few drops of blood (similar to testing for diabetes)

For all anticoagulants, labs will be reviewed during the visit, and any adjustments will be made to your warfarin regimen by the pharmacist caring for you and also communicated to your referring provider.

Our pharmacists will work with you to:

Understand your diagnosis and the role of anticoagulation in it

  • Understand your diagnosis and the role of anticoagulation in it
  • Know the signs and symptoms of thromboembolism.
  • Understand bleeding risks associated with these medications and how to manage them.
  • Feel educated about your anticoagulant and the potential interactions of other medications you are on.

Request your Physician that prescribed your anticoagulant to refer to our pharmacist for management.

Smoking Cessation

The center offers smoking cessation management, including prescribing medications. Our smoking cessation-trained clinical pharmacist will help you make this important step in controlling your health. Treatment for smoking and tobacco use is a patient-centered approach, which considers the patient’s desire and motivation to quit, and preference for treatment, of which the combination of medication and counseling is preferred.  All appointments are patient-specific (one-on-one).

Hypertension management

If you need help managing your high blood pressure, the Mount Carmel Pharmacy Outpatient Care Center is your resource for expert, collaborative care and personal service.

Our pharmacists will partner with you so you best understand lifestyle changes that you can make to best impact your Hypertension and get it under control.

Lifestyle changes in combination with titration of medications can help get your blood pressure to the goal numbers that will help lower your risk of serious and life-changing consequences of uncontrolled high blood pressure.

The center provides one-on-one consultations with a pharmacist under the guidance of our hypertension medical director.

If you think a consultation at the Mount Carmel Outpatient Care Center may be helpful to you, you’ll need a physician referral and to meet the following eligibility requirements:

  • Be at least 18 years old
  • Not be pregnant
  • Not already be under the care of a nephrologist
  • Not have complex congenital heart disease or advanced kidney disease

Obtained his Doctoral Degree in Pharmacy from Lake Erie College of Osteopathic Medicine. His special interests include diabetes and Medication Therapy Management, Smoking cessation, and hypertension management.

James Pantelis, PharmD

Obtained his Doctor of Pharmacy Degree from The University of Toledo, then went on to complete his Board Certification in Ambulatory Care from the Board of Pharmacy Specialties. His professional interests include Anticoagulation, Hypertension, Smoking Cessation, and Diabetes.

Obtained his Doctoral Degree in Pharmacy from The Ohio State University, then went on to complete a PGY-1 Pharmacy Residency at Mount Carmel West Hospital and a master’s degree in business administration from Mount Vernon Nazarene University. His special interests include Leadership, Diabetes, Medication Management, Cardiovascular diseases, Wellness/Preventative Medicine.
