Diabetes Management

Managing diabetes can be complex. Many people find it a lot to handle without guidance. That’s why Mount Carmel created the Diabetes Self-Management Program. It’s a collection of classes and consultations, plus a support group, that put you in touch with the demands of your disease and the people who can help you manage it.

The program gives you the tools to control your diabetes — so it doesn’t control you.

In addition to offering education and instruction, the Diabetes Self-Management Program introduces you to others who are dealing with the disease and living well. Taking part in the program gave them more control of their illness and prepared them to cope with the disease. It can do the same for you.

The Diabetes Self-Management Program at Mount Carmel is certified by the American Association of Diabetes Educators and meets the National Standards for Diabetes Self-Management Education.

Our comprehensive self-management program will provide you with information that will help you manage your disease with confidence. Classes are taught by registered nurses, pharmacists and dietitians, and are small enough so that you'll get plenty of individual attention.

The first 3 sessions are held one week apart, and the final session is scheduled four weeks later. For your convenience, both daytime and evening classes are offered. The cost is usually covered by insurance. A support person is welcome to attend with you at no additional charge.

Session 1:

In the first class a diabetes educator will meet with you individually. You will receive information about your own health risks. You’ll also learn about diabetes medications and blood sugar monitoring.

Session II:

The second class is taught in a small group and you'll find out what diabetes is, why it happens and what lifestyle changes you should make.

Session III:

In the third class, you’ll have had a chance to try carbohydrate counting. You’ll get in-depth information on how to make better food choices, special considerations for when you are ill, and how to start an exercise plan. You’ll have help setting up a Success Plan for making these important changes.

Session IV:

The fourth session is held about 4 weeks later where you will review your Success Plan with the help of your instructor.

Not everyone with diabetes uses an injectable medication. For those who do, we’ve created a training program that covers everything you need to know about this important, effective therapy.

If you’re new to insulin, this training program is an ideal way to learn about its benefits and effects. It is generally conducted one-on-one. Your session will cover a host of topics, including:

  • Why injectable medication therapy is necessary
  • How to give an injection
  • The importance of site rotation
  • Injectable medication therapy action times (onset, peak, and duration)
  • The importance of timing injections with meals
  • The importance of keeping blood sugar records to monitor progress
  • When to report problems to your physician

Private consultations also are available for diabetes patients with immediate or special needs. You can schedule an appointment if you or someone you’re caring for:

  • Has Type 1 diabetes
  • Is unable to attend the Type 2 Basics class
  • Has been diagnosed with gestational diabetes
  • Has a vision or hearing impairment
  • Needs the help of a translator
  • Suffers from mild mental retardation or a developmental disability
  • Private consultations cover many of the same topics as our comprehensive program, but the sessions are designed specifically to meet your individual needs.

Private consultations are available during normal business hours and are usually covered by insurance.
