At this time, Mount Carmel is scheduling COVID-19 vaccines for patients ages 5 and older. If you would like to schedule an appointment for a patient that is younger than 5 years old, please visit the Ohio Department of Health's website for availability.

Getting a COVID-19 vaccine is one step you can take to keep you and your family safe. At this time, Mount Carmel is scheduling appointments for eligible individuals. Eligibility details and appointment scheduling are below. COVID-19 vaccine is also available at select retail clinics and health providers throughout the state. Mount Carmel offers Pfizer and Moderna COVID-19 vaccine appointments. Vaccine Appointment FAQs

All people who are 17 years of age or younger must have a parent or legal guardian at your appointment in order to receive their vaccination. Minors do not need to provide a photo ID.

If your appointment is at a Pharmacist Outpatient Care Center, please call 614-234-8844 to reschedule. If your appointment is at a Mount Carmel Medical Group location, please call that location where you are scheduled.
