Ways to Give: Donor-Advised Fund (DAF)
Give Through Your Donor-Advised Fund (DAF)

A donor-advised fund (DAF) is a simple, tax-efficient way to support the Mount Carmel Foundation. If you have established a DAF through a community foundation, financial institution, or charitable sponsoring organization, you can recommend a grant to benefit the Foundation and help advance our mission.
How to Make a Gift from Your Donor-Advised Fund
- Contact your donor-advised fund administrator.
- Recommend a grant to:
Mount Carmel Health System Foundation
5955 East Broad Street
Columbus Ohio 43213
Tax ID (EIN): 31-1113966 - Indicate the area or program you would like your gift to support or allow your gift to be used where it is needed most.
- Notify us of your grant recommendation so we can properly acknowledge your generosity.
Donor-advised fund grants may be used to fulfill charitable giving intentions but cannot be used to satisfy legally binding pledges or provide benefits to the donor, in accordance with IRS regulations.
