Meredith Mucha, MD, MHA

Program Director

  • Dr. Mucha obtained a BS in Biochemistry from Davidson College and a BSE in Chemical Engineering from Columbia University.
  • She completed her Doctorate of Medicine from Wright State University, residency in Internal Medicine at Riverside Methodist Hospital, fellowship in Geriatric Medicine at The Ohio State University Wexner Medical Center, and Master’s in Healthcare Administration from LSU.
  • Dr. Mucha is board-certified in Internal Medicine, Geriatric Medicine, and Hospice & Palliative Medicine. She was previously the Geriatric Medicine Fellowship Program Director at Ohio State, Medical Director for Capital City Hospice and Acclaim Hospice, Medical Director at several nursing facilities in Columbus, and an inpatient palliative care consultant at The James Cancer Center of Ohio State.
  • As an advocate for the elderly underserved population, Dr. Mucha has worked to create integrated Geriatrics healthcare systems and service lines within central Ohio and is excited to grow Geriatrics at Mount Carmel, which currently includes the fellowship, outpatient Geriatrics Clinic, and inpatient consults. She also hopes to expand Geriatrics with Mount Carmel’s Population Health via upstream, senior community coordination and certify Mount Carmel Health as an Age-Friendly Health System with the Institute for Healthcare Improvement.
  • She has presented nationally at MD Anderson regarding rehabilitation capacity in palliative care patients, authored American Geriatrics Society’s CoCare®: Ortho (Geriatrics-Orthopedics Co-Management model) in pre-op and post-op pain management, and guest speaks to Ohio University graduate bioscience students.
  • She is active with World Vision and enjoys spending time with her husband and son, napping with her dogs, doing outdoor sports, watching hockey (go Blue Jackets!), and shuttling her son and his friends to their sporting events around the Midwest.

Matthew Chakan, MD, FACP

Matthew Chakan, MD, FACP

Associate Program Director

  • Dr. Chakan received his Bachelor of Science in Bioengineering from the University of Pittsburgh and his Doctor of Medicine from Boston University. He also completed his Internal Medicine residency at Boston University.
  • Dr. Chakan is Board-certified in Internal Medicine.
  • After working as an outpatient Primary Care Provider for Allegheny Health Network in the suburbs of Pittsburgh, Pennsylvania, Dr. Chakan returned to his passion of medical education.
  • Dr. Chakan was an Assistant Professor of Medicine and core faculty member of the Eastern Virginia Medical School Internal Medicine Residency Program in Norfolk, Virginia. He chaired several resident education and development committees and received numerous teaching awards.
  • Dr. Chakan is interested in teaching and practicing Evidence-Based Medicine, caring for the underserved, and developing physicians-in-training using Evidence-Based Medical Education and Adult Learning models.
  • Outside of work, Dr. Chakan enjoys traveling, exploring new restaurants, kayaking, and skiing.

Lyle Olson, MD

Lyle T. Olson, MD, FACP

Faculty

  • Dr. Olson received his B.S. in biology from The Ohio State University.
  • He received his medical degree from The Ohio State University and completed his residency in internal medicine at The Ohio State University Hospitals.
  • He has cared for nursing home patients in various capacities during all his practice years, committing full-time to their care since 2016 as part of MedOne Healthcare Partners.
  • He serves as the medical director of several skilled nursing facilities.
  • He and his wife enjoy music, hiking in the national parks, and visiting their grown daughters at various locales.

 

Neil Richard, MD, FAAFP

Faculty

  • Dr. Neil Richard of Central Ohio Primary Care is the medical director of Worthington Christian Village and Willow Brook Christian Home.
  • Dr Richard grew up in Worthington and graduated from Otterbein College. He earned his medical degree from The Ohio State University College of Medicine and completed his Family Practice residency at Grant Medical Center.
  • Dr. Richard is board certified in Family Practice, is a member of the American Board of Family Practice, and a Fellow of the American Academy of Family Physicians.
  • He has been recognized by the National Committee of Quality Assurance for his excellence in the care of the diabetic and stroke patients.
  • He also donates his time to the Free Clinic and the teaching of medical students.

Hannah Parsons

Hannah Parsons, BS

Program Administrator

  • Hannah is the Program Administrator for both the OB/GYN and Geriatrics programs. She is excited to help residents and fellows navigate through their time here at Mount Carmel.
  • Hannah is from West Jefferson, Ohio and graduated from Ohio University.
  • She enjoys spending time with her fiancé, friends, and family, going to sporting events, and playing cornhole.
