Street to Housing Outreach Collaborative

Mount Carmel’s Street to Housing Outreach Collaborative (SHOC) addresses the physical, mental health & social care needs of our community members experiencing homelessness in Columbus, OH and Franklin Co. by improving the access, quality and efficiency of services. This includes:

• Facilitating inter-agency communication to promote increased and timely continuity of care
• Assessing community needs and evaluating the services available to community members
• Implementing a critical case management process for the most vulnerable patients
