Complete Application packets should be completed through VSLO. 

* Please note that we require an active Education Affiliation Agreement with your institution before we can approve your rotation. We ask that you give us at least 90 days to fully execute the agreement. If the agreement cannot be completed at least 45 days in advance of your rotation you will not be permitted to rotate with MCHS/MCMG.
