Drug and Health Screen
IMPORTANT: You should make every attempt to attend your scheduled appointment. If for some reason you are unable to, you must call Employee Health 380-898-5589 and email your recruiter or TA Coordinator (information in your Welcome Email) to cancel/reschedule. If you cancel, you have 1 business day to reschedule. If you no call no show to your appointment, your offer will be automatically rescinded.

Specific instructions:
- Arrive 10 minutes prior to your scheduled appointment time
- Please do not bring children to the appointment due to the sensitivity of the screening
- The process will take approximately 1 hour
- Your health screening will include a urine drug screen and blood draw
- Fasting is not necessary for your appointment
- You will be taking your badge photo during your appointment, be camera ready. **You will receive your physical badge during this appointment. You’ll be instructed to go to Security after this appointment to pick your badge up. This process excludes Mount Carmel New Albany and Diley Ridge Medical Center. You’ll receive your badge on your 1st day onsite.
- Note: New candidates under the age of 18 years old must bring a parent or guardian to the appointment for witnessed consent.
Bring the following documents with you:
- Photo ID
- Your completed and printed health screening paperwork from Workday. Click here if you need the screening form.
- Documentation of the following vaccinations/immunizations. Proof of immunization documents must include:
- Varicella (chicken pox) — documentation of 2 doses of the vaccine or lab documented proof of immunity
- Influenza (Flu) — proof of vaccination from current season
- Mumps, Measles & Rubella — documentation of 2 doses of the vaccine or lab documented proof of immunity
- Pertussis — documentation of Tdap vaccine
- Hepatitis B (for jobs prone to exposure to blood) — documentation of 3 dose series or lab documented proof of immunity
- In the event that prior medical records are required, please bring contact information for your previous employer(s) or treating physician.
- Prospective colleagues are required to complete all required testing and/or vaccinations initiation as a condition of employment. Failure to comply with the immunization requirements will result in the offer of employment being withdrawn.
