Continuity of Care

Certain patients, including those who are pregnant or undergoing active treatment for a chronic condition, may qualify for extended in-network access through Cigna’s continuity of care program. If you think you may qualify, immediately call the number on the back of your Cigna health insurance card, and download, complete, and send this Transition/Continuity of Care form to Cigna (address is included on page 4).

NOTE: Transition/Continuity of Care requests will be reviewed within 10 days of receipt. For new Cigna customers, review will occur within 10 days of participant's effective date. Review for Organ Transplant requests may take longer than 10 days.
