Application Process and Eligibility

Mount Carmel participates in the match program of the American Society of Health-System Pharmacists and agrees that no person at this site will solicit, accept or use any ranking-related information from any residency applicant. To learn more about this program, visit www.ashp.org.

Eligibility

Applicants to the Mount Carmel Health System Residency Program must be graduates or candidates for graduation of an accreditation council for pharmacy education (ACPE) accredited degree program (or one in the process of pursuing accreditation) or have a foreign pharmacy graduate equivalency committee (FPGEC) certificate from the national association of boards of pharmacy (NABP). At a minimum, the program must be a five-year pharmacy degree program. The Mount Carmel Health System is unable to sponsor a VISA.

Applicants to the Mount Carmel Health System Residency Program must be licensed or eligible for licensure in the state of Ohio.

Pharmacy licensure in Ohio is a requirement for all residents at Mount Carmel Health System. Prior to the residents start date, at a minimum the resident is required to have an active Ohio Pharmacy Intern License. It is preferred the resident has an active Ohio Pharmacist License prior to the residency start date. All residents are expected to obtain licensure within 2 months from the residents start date. If a resident fails to obtain licensure within this time period it may result in dismissal from the program and termination of employment. The Residency Program Director and the Pharmacy Director will review these situations on a case by case basis with emphasis on cause of delay and agree upon an outcome. Failure to obtain a pharmacist license within 90 days of the start date of the residency will result in dismissal from the program and termination of employment. Per ASHP Residency Standards, a minimum of 2/3 of the residency program must be completed as a pharmacist licensed to practice in the state of Ohio.

For application to PGY-1 Pharmacy Practice Residency interested candidates should submit application via PhORCAS™ (Pharmacy Online Residency Centralized Application)

    • Pharmacy Transcripts
    • Three Letters of Recommendation

In PhORCAS, there is one application for both sites. In the NMS match, you will rank each site separately.
