Care Coordination Rate = In Network Utilization = Enhanced Patient Care

At Mount Carmel Health Partners (MCHP), one of our top priorities is to ensure that our providers keep our patients within the Clinically Integrated Network (CIN). Care Coordination Rate (also known as "In Network Utilization") will continue to be a focus. By providing patient care within our network, patients experience optimized care and better quality outcomes. With over 2400 providers, patients have many options to receive care from a provider within the Mount Carmel CIN.

The goal of clinical integration is to provide your patients with high-quality, cost-effective, coordinated and innovative care. We want to ensure continuity of care in all settings through our network of providers and Mount Carmel facilities. When a patient utilizes providers or services outside of the network, not only does it become more difficult to provide them with the wide spectrum of services available, the provider also loses their line of sight on the patient.

MCHP is working hard to offer our providers timely and accurate data related to Care Coordination Rate. We would love to share this data with your practice.
