Important Notice to All Practitioners Providing Laboratory Services


Practitioners who perform in-office laboratory testing must provide a copy of their Clinical Laboratory Improvement Amendments (CLIA) certificate, corresponding CLIA number and all laboratory locations on their Council Affordable Quality Healthcare (CAQH) credentialing application. Please review and update the CLIA information when re-attesting to your CAQH application. Payer contractual obligations require Mount Carmel Health Partners to have all current CLIA certificates on file at the time of initial credentialing and for ongoing monitoring of certificates. Providing updated information also assures that our contracted clients receive the most up-to-date practitioner data.
