Share Your Gratitude

Have you or a loved one received exceptional care at a Trinity Health hospital, a medical center, or a Trinity Health provider’s office? We invite you to share your gratitude.

You may honor any colleague, nurse or provider by sharing your gratitude here. Recognized colleagues may be considered for one of our award programs, such as Heart of Trinity Health, DAISY Award (for RNs), Friend of Nursing Award, Physicians Making a Difference, or the Anchor and Lighthouse Awards.

Your submission will be reviewed by the appropriate awards committee. If you wish, you may also make a charitable gift to honor your caregiver through our Grateful Patients & Families Program.
