COVID-19 Vaccine Scheduling for Ages 5-11


At this time, Mount Carmel is scheduling COVID-19 vaccines for patients ages 5 and older. If you would like to schedule an appointment for a patient that is younger than 5 years old, please visit the Ohio Department of Health's website for availability.

Vaccine Appointment FAQs

If an individual is 5 to 11 years of age, they are only eligible to receive the Pfizer COVID-19 vaccine at this time. They must have a parent or legal guardian at their appointment in order to receive their vaccination. Minors do not need to provide a photo ID.

If you received your vaccine at an Anticoagulation Center, please call 614-234-8844. If you received your vaccine at a Mount Carmel Medical Group location, please call the location where you are scheduled.
