Congratulations! You've taken the first step in pursuing this life-changing surgery by either attending a seminar or by watching our educational video. If you feel you are eligible for bariatric surgery, the next step is to complete and submit the bariatric pre-screening application. Your health and well-being are our primary concerns, so you also will receive an extensive medical evaluation to determine if bariatric surgery is safe and appropriate for you.
