Providers

Services offered by referral accepting Collaborative Practice Agreement (CPA) with the Pharmacist team at the MCHS Pharmacy Outpatient Care Center (POCC). Following internal policies outlining CPA details and practice guidelines approved by Physician Medical Directors of the service, Dr. Meredith Mucha, MD, and Dr. Michael Meleca, MD FACC, for Hypertension.

Services offered include:

Anticoagulation Management

  • Warfarin
    • Perform drug therapy management for warfarin to treat the disease state on the referral. Includes in-office monitoring of PT/INR and any adjustments to manage the patient in range.
    • Managing Bridging patients when warfarin needed to be held for procedures, surgery, or other reasons indicated by providers.
    • Ordering any pertinent lab values to assess patients’ anticoagulation status.
    • Manage the transition to warfarin from another anticoagulant or from warfarin to another anticoagulant.
    • Educate patient about disease state(s) needing anticoagulation.
    • Educate the patient on compliance of medication.
    • Education the patient on the signs and symptoms of bleeding and thrombosis.
    • Manage all refills and prescriptions for warfarin and Low Molecular Weight Heparin (LMWH) for bridging.
    • Communicate to referring provider with every visit.
    • Communicates with providers with every visit.
  • Direct Oral Anticoagulant (DOAC)
    • Educate patient about disease state(s) needing anticoagulation.
    • Educate the patient on compliance of medication.
    • Education the patient on the signs and symptoms of bleeding and thrombosis.
    • Manage all refills and prescriptions.
    • Manage medications around times DOAC needed to be held for procedures, surgery, or other reasons indicated by providers.

Smoking Cessation

  • Utilizing the Smoking Cessation Standard of Care based on the American Lung Association, The Centers for Disease Control and Prevention (CDC), Association for the Treatment of Tobacco Use and Dependence (ATTUD).
  • Treatment for smoking and tobacco use is a patient-centered approach, which considers the patient’s desire and motivation to quit, and preference for treatment, of which the combination of medication and counseling are preferred.
  • Managing ordering and refilling, if needed, of Nicotine Replacement Therapy (NRT), Bupropion, or Varenicline to help support the patient.

Hypertension management

  • Collaboration of care to augment the management of your hypertension patient.
  • Another health care provider touchpoint to get accurate Blood Pressure reading in the medical record.
  • Partnering with the patient to educate them on their Hypertension and risks associated with noncompliance.
  • Utilizing the AMA Map BP program guidelines to act fast to titrate the patients’ medications to get them to the target blood pressure quickly and safely.
  • Educate the patient on their medications and why they should be compliant with them.
  • Assess for adverse drug reactions and adjust therapy accordingly to avoid them.
  • Order and assess lab values to ensure medications are appropriate.
  • Education on diet and sodium intake to assist with their blood pressure treatment plan.
  • Managing ordering and refilling of hypertension medications.
  • Communicate to the referring provider with every visit so you are always aware of the care being provided.
  • The referring provider is leading this treatment and can cancel the referral at any time or request to manage any aspect of the care themselves if preferred.
