When it comes to heart failure, Mount Carmel's goal is to minimize the amount of time you spend in the hospital and maximize your ability to manage your disease. That’s why we offer personal, professional heart failure services that ensure you receive the care you deserve and the peace of mind that comes from knowing someone is looking out for you at all times.

Our comprehensive heart failure program includes education, personal care and access to a host of helpful resources, like:

Heart Failure Coordinators

Our heart failure coordinators are experienced cardiac nurses who work with you, your family, your caregivers and your physicians to facilitate treatment based on your needs.  In addition to reviewing your medical history and identifying changes in your condition and treatment, they make sure you have access to the education and resources you need.

Inpatient Care

By grouping our heart failure specialists and technologies together into Heart Failure Units at Mount Carmel East, Grove City and St. Ann’s, we give you the most direct care possible and surround you with expert professionals who understand heart conditions and symptom management.

Outpatient Care

Mount Carmel is dedicated to helping you learn to live with heart failure by providing access to outpatient heart failure services at three locations throughout central Ohio. Under the direction of a cardiologist, the experienced advance practice providers at each location can provide you with education, prescribe and monitor your medications, and offer regular follow-up to keep your symptoms under control.

  • Helpful Heart Failure Tip: Heart failure can be very manageable with activities such as being consistent with medication management, reducing sodium intake, monitoring weight changes, staying active within your limits, avoiding smoking and alcohol, and managing stress.
  • Special Hobbies: Working out, traveling, watching my kids play sports, cooking, and entertaining friends and family.
